Do You Need an Endocrinologist, or Can Your Family Doctor Manage Your Diabetes?
When you’re diagnosed with diabetes, one of the first questions that comes up is: who should you see for ongoing care? Some people assume they need an endocrinologist; a specialist in hormones and the endocrine system. Others see their family doctor and aren’t sure if that’s enough.
The honest answer: for most people with type 2 diabetes, a family doctor is exactly the right choice. Here’s how to know which situation applies to you.
What endocrinologists do
Endocrinologists are physicians who specialize in conditions involving hormones and glands. That includes diabetes, but also thyroid disorders, adrenal disorders, pituitary disorders, osteoporosis, fertility issues, and several rarer conditions.
Because they focus specifically on these areas, endocrinologists are valuable for complex or unusual hormone-related cases. They tend to have deep expertise in advanced diabetes technology (insulin pumps, continuous glucose monitors), the newest medications, and complicated cases that don’t respond to standard treatment.
What family doctors do
Family doctors, also called primary care physicians or family medicine doctors are trained to diagnose and manage the most common conditions across a person’s entire life. That includes high blood pressure, cholesterol, diabetes, depression, common infections, and preventive care.
Crucially, family doctors are trained to manage type 2 diabetes from diagnosis through long-term care. They handle:
- A1C testing and diagnosis
- First-line and second-line medication (metformin, GLP-1s like Ozempic and Mounjaro, SGLT2 inhibitors, and others)
- Insulin therapy when needed
- Coordinated management of blood pressure, cholesterol, and weight, all of which interact with diabetes
- Annual screenings for diabetes complications (eyes, kidneys, feet, heart)
- Lifestyle and nutrition guidance
For the majority of people with type 2 diabetes, especially those whose A1C is reasonably under control this is all the care they need.
When a family doctor is the right choice
If any of these describe your situation, your family doctor can almost certainly manage your diabetes:
- You have type 2 diabetes
- Your A1C is below 9% (and especially if it’s below 8%)
- You’re on oral medications, GLP-1s, or basic insulin
- You don’t have complex complications (severe kidney disease, frequent hospitalizations from blood sugar swings)
- You want one doctor who knows your full health picture, not separate specialists for separate parts of you
The advantage of staying with a family doctor is continuity. Your doctor knows your family history, your stress levels, your other conditions, and how all of it connects. Diabetes doesn’t exist in isolation and neither should your care.
When an endocrinologist makes sense
A referral to an endocrinologist is worth considering if:
- You have type 1 diabetes (specialist care is typically standard)
- Your A1C remains above 9% despite multiple medication adjustments
- You’re pregnant or planning pregnancy with pre-existing diabetes
- You’re using or considering an insulin pump or advanced CGM system
- You have a complex hormone issue alongside diabetes (severe thyroid disease, pituitary issues, adrenal disorders)
- You’ve had repeated hospitalizations for diabetic ketoacidosis or severe hypoglycemia
- Your family doctor specifically recommends a referral
Even in these cases, you typically keep your family doctor as your primary provider. The endocrinologist becomes a partner handling specific aspects while your family doctor still manages the rest of your health.
The cost and access difference
This matters too. Endocrinologists are in short supply almost everywhere in the U.S., and the wait for a new patient appointment is often 3–6 months. Specialist visits also typically cost more per visit and may have higher insurance copays.
Family doctor appointments are usually available within days, cost less per visit, and are covered as preventive or chronic care under most insurance plans. If you’re in the Memphis/Cordova area and want diabetes care started quickly, your family doctor is almost always the faster path.
